NATUROPATHIC OPTIMAL WELLNESS OFFICE FEES

NEW PATIENT FEE $425 -$475 FOR A 60-90 MIN APPOINTMENT – Includes thorough history & intake, indicated physical exams, recommended labs and/or evaluation of labs submitted before visit.

FOLLOW UP OFFICE CONSULTS VISITS ARE BASED ON TIME SPENT AND COMPLEXITY

• 10-15 MINUTES $85-$125
• 15-20 MINUTES $100-$135
• 20-30 MINUTES $120-175
• 35-45 MINUTES $190-$250
• 50-60 MINUTES $265-$295
• 65-75 MINUTES $310-$350

*MOST FOLLOW UP VISITS RANGE FROM 30-60 MINUTES

PROLOZONE NEW PATIENT VISIT FEE $450.00

Includes 45-60-minute history, intake, physical exams and lab review, nutrient recommendations plus additional 20-30 minutes for first prolotherapy and ozone injection if appropriate. If deemed not to be appropriate at this time, you will only be charged $350.00 and eligible for established patient follow up visit fee rates for future visits.

PROLOZONE FOLLOW UP VISITS

• 30 MIN $200.00 INJECTION IN ONE MAJOR AREA (IE LOW BACK) OR ONE JOINT (KNEE, ELBOW, ANKLE, WRIST, FOOT ETC.)
• 10 MIN ADD ON SECOND MAJOR AREA $150-$175.
• 5 MIN ADD ON MINOR AREA: SUB-Q OR SINGLE PRESSURE POINT $75-$125.

SERVICE CHARGES FOR PROCEDURES

• NECK RELEASE PROCEDURE: ACUPRESSURE/MASSAGE/NATUROPATHIC MANIPULATION $75-$95 (20-30 MINUTES)
• OTHER THERAPEUTIC BODY WORK $45-$180
• PROLOZONE AND OZONE THERAPY ADD ON FEES WITHIN APPOINTMENT
  • Ozone therapies: Insufflation: $20-$30 per treatment – there may be additional cost of $5-10 for equipment needed.
  • Ozone bagging treatment: $25-60.
  • Prolozone Injections (allow 20-30 minutes) $85-$200.
• HYPNOSIS WITHIN APPOINTMENT OR INDIVIDUAL SESSION
  • 20-60 MINUTES $45-$150
